No.	DESCRIPTION OF COMPENSATION	BENEFITS
1	DEATH Total Sum Insured for Death as per Schedule of Benefits The sum payable under Death benefits shall be deducted by the amount that has been paid and or shall be paid for Permanent Disablement of the same accident (or the same series of accident)	US.$ 25.000
2	PERMANENT DISABLEMENT	US.$ 25.000 (max)
3	MEDICAL EXPENSES The insures will reimburse customary and necessary reasonable Medical Expenses up to the maximum amount payable as state in the Schedule of Benefits	US.$ 2.500
4	REPATRIATION EXPENSES The insurer will pay this expenses up to the maximum amount payable as state in the Schedule of Benefits when or as the result of an accident whilst in Indonesia-the Insured Person died within thirty (30) days from the date of the accident. The insurer's Assistant or its or its authorized representative shall make the necessary arrangements for the run of the Insured Person's mortal remain to his/her country or any location as may be selected by the Insured Person's legal representative. In addition to the transportation of the remains, the insurer shall reimburse the Insured Person's expenses actually incurred, for services and supplies provided by mortician or undertaker, including but not limited to the cost of a casket, the embalmbing and cremation if so elected, inclusive in the state amount of the Schedule of Benefits. 5	EVACUATION EXPENSES The insurer pay this expense up to the maximum amount payable state in the Schedule of Benefits when or as the result of an accidents whilst in Indonesia – and if in the opinion of the Insurer's Assistents or its authorized representative it is judged medically appropriate to move the Insured Person to another location for medical treatment or to return the Insured Person to his/her country or to any location as may be selected by the Insured Person's legal personal representative, the Insurer's Assistant or its authorized representative shall arrage for the evacuation utulizing the best means suitd to do so based on the medical severity of the Insured Person's condition. 6	OVER STAY EXPENSES The insurer will pay for overstay benefits caused by the occurrence of natural disaster & accidents in order to comply with legal statutes, at payment above the amount state in Section 1,2,3,4 or 5 which include the expenses of medication, hotel,etc.	US.$ 1.000

Claim Prosedure

• Once any loss or accident takes place, the insured must contact our nearest Branch Offices immidiately
• The insured have to give notice of claim in writing to the insurer
